ICSP and Reset chips

When using a PIC with VPP and MCLR on the same pin, does anyone use the supervisory chips like a MAX6585 or MAX806 ? Those MAX chips only tolerate +6V on the open collector output so I suspect they would blow when VPP is applied to the MCLR pin for programming. Do I have to use jumpers or is there another family of supervisory chips I can use? Or does everyone just use a resistor to VCC?
