You'll have to say what version of PCM and what version of MPLAB. It's taken me two long, long days, with help from PCMProgrammer to get my older PCM 2.6xx 'linked into' MPLAB 5.70.
Seems the newest MPLAB does NOT like older PCM.
You'll have to get the 'setup' software from CCS for your version of PCM as well.
I've got about 50 PCM builds without problems and about 20 ASMs as well. No blue screens of death..hmm..it actually is pleasant, again!

If you have a modern version of the CCS compiler (last three years or
so) then CCS has a tutorial on how to setup the compiler with MPLAB,
complete with screenshots:
http://www.ccsinfo.com/faq.php?page=ccs_mplab
Or, if you have MPLAB X, see this tutorial:
http://www.ccsinfo.com/pdfs/mplabx_ccs_tutorial.pdf

You can download the MPLAB "plug-in" from this page:
