I want to create bootloader. How to begin?

I want to create my first bootloader project, but I don't know how to begin.
Can I load the controller with a bootloader program for a very first time, or I have to program it with a conventional programmer (pickit) and then to update the firmware with bootloader. In other words what are the first steps I have to make to create a simple bootloader project via rs232(serial port).
