flex_lcd driver and 18F4620

I am having a devil of a time with a simple task. Using a known good combination of a 2X16 LCD (JHD 162A) and an 18F4620. I just cannot make this work with a PCWH compiler. Has anyone used this combo? Pin assignments are:

Using PICKIT 2 programmer - works fine with flash LED program

Obvious first comment. You have got the R/W pin on the LCD pulled low?.
However most likely thing is what else PortD is used for. Have you got the line:

setup_psp(PSP_DISABLED);

in your initialisation?.
