Newbie Programming and Layout Questions

When I make my board, what is the best way to program the PIC? Have a port for my ICD-U40 like the demo board. Or is there a way to do it over the USB port, as I am putting on my board as well.

If I use the ICD-U40 like my demo board, it uses the SPI DI DO hardware pins. Can I just use a wired AND there, or will I need some kind of protection for my SPI bus?

If you can point me to a good source of information for someone using a PIC on their own design for the first time, that would be great.

For a first board I would avoid using a bootloader, and just use dedicated ISP programming pins. Read the section of the datasheet about ISP (In circuit Serial Programming).
