ICD-U40 not programming the chip

Hi,
I am unable to download the program in PIC18f252 from my ICD-U40.
When i try to download the program the error occurs and gives the detail lke that

17:23:50 Assigning chip: PIC18F252
17:23:50 Target Vdd = 5.16
17:23:50 Target Vdd = 5.13
17:23:50 Write FILE -> CHIP
17:23:51 Download failed - Chip Detect.

I am using CCSLoad utility to download the program from ICD-U40.
How to check my ICD-U40.

Did you go into the Diagnostics page on the app and test
the 3 outputs at VSS and VDD ?

My ICD's have a fault with one of the line stuck at VDD - this causes the same error you have ( blown driver in the ICD).
