ICD-S40 verification Problem

I am using ICD-S40 with compiler CCS PCWH 4.074. we have a verification problem always on the screen, after downloading .hex code to the microcontroller 18F4620. How can i solve this problem_? I try to update the firmware, but there is no change.

There is no problem about truely working. Only there is one thing that is voltage surge. If you put the 100nF capacitor between +5v and Ground, ICD-S40 is working truely. So, verification problem is getting out.
