icd-u40 and 18f452 problem

have a board with an 18F452 on it. I have been using a low voltage programmer to program it, so B5 was pulled low and I had LVP in the #fuses.
I just got the ICDU-40 and used it on a usb dev board so the ICD is ok. On my board I removed the pulldown on B5, and made up a cable according to ICD manual. I did not connect B3, just B6,B7, gnd, 5V and Mclr. I just double checked the wiring and am pretty sure it is correct. When I run the ICD program, it passes comm test and ICD test, but fails the target test saying it could not detect the target chip. the icd program is V2.61. ICD FW is 2.09.
With the ICD connected my board continues to run until I start the ICD program then it stops the board. ANy ideas on why it does not recognize my chip?

Go through the flow charts and see if they can tell you the problem.

going through the manual stuff checks out till I get to the part where I measure Mclr, it should be 13V but I get 5. I guess this is the problem
