CCS C USB development kit

I used the CCS C compiler whose version is 3.221. Moreover, I have had the CCS C USB development kit. In this kit I made the ex-9. It is ok. communication and analog value and switch value can be seen at delphi interface on my pc. My problem is that, I made my board using the same code (with same pic18f4550) with same pin connection C4 to USB_Pin2 and C5 to USB_Pin3. Windows XP say that unknow usb device. What can be reason???????

Moreover,

when I use the the CCS C USB development kit , After download the code with using debugger with ICD-U40 , device is ok and running until I disconnect the ICD-U40 . why?? because the usb port still connected.
