Can't chip erase PIC18F452

Every time I try to use the ICD Interface to erase an 18F452, it says, "Chip erase complete." Then, when I verify the chip is blank, I find the program still there. Same with "Program Memory Erase." There isn't any kind of code protection in place. Is this a problem with the ICD Interface?

What is the Vdd voltage of the PIC on your board ?
